Title:Cervical branch of the facial nerve in leprosy.

Abstract:This study demonstrates that the platysma is occasionally palsied in leprosy and that this only occurs when the facial nerve already has some other palsy. That there needs to be a facial palsy before there can be a platysma palsy is strongly suggested, in that there was no case of an isolated platysma palsy. Patients, regardless of age or other factors, could mimic a platysma contraction. This obviates the need for electrical testing to examine for a platysma palsy. It also means that a nonfunctioning platysma on clinical examination is, in fact, a palsied platysma. While lagophthalmos is regularly examined for, and any obvious facial paresis would be noticed, less severe forms of facial muscle paresis will only be found if formally examined for. The mechanism whereby the facial nerve is involved in leprosy is not clarified, but our findings suggest that proximal spread of a lesion that began in the zygomatico-temporal branches and reaches to the facial nerve trunk is more likely than new lesions developing de novo in other peripheral facial nerve branches. That the primary lesion is within the facial nerve trunk in all cases but we only see the frequent zygomatic sequelae due to secondary factors is not excluded. (AU)^ien.

Title:Auditory brain stem evoked potentials in patients with leprosy.

Abstract:Nineteen, randomly selected male patients with lepromatous leprosy were evaluated electrophysiologically. All of these patients had long-standing disease and were treated with dapsone alone. There were statistically significant differences between the values obtained in this group of leprosy patients compared to 20 age-matched controls in auditory brain stem evoked potentials (ABEP). The findings are consistent with a pathologic process located mainly between the cochlear nucleus and the lateral lemniscus in the auditory brain stem pathways. It should be emphasized that our patients had long-standing disease which was treated with dapsone. ABEP could very well be different in leprosy patients diagnosed early and treated for relatively short periods with multidrug therapy. Brain stem evoked response audiometry may be useful for evaluating the possibility of brain stem involvement in leprosy. (AU)^ien.

Title:Anti-neutrophil cytoplasmic antibodies (ANCA) in the clinical forms of leprosy.

Abstract:Anti-neutrophil cytoplasmic antibodies (ANCA) are autoantibodies against enzymes present in primary granules of neutrophils and lysosomes of monocytes detected in systemic vasculitis and in other diseases, including infections. ANCA are markers of active Wegener granulomatosis, which presents some anatomo-pathologic and immune response features similar to those of leprosy. Thus, we raised the hypothesis that ANCA may be present in leprosy as markers specifically linked to the presence of vasculitis. The aim of this study was to determine the presence of ANCA in leprosy and its correlation with the clinical forms of the disease. Sera from 60 normal individuals and from 59 patients with different clinical forms of leprosy were studied. The patients were also allocated into reactional and nonreactional groups. By indirect immunofluorescence, ANCA were positive, an atypical pattern (A-ANCA), in 28.8% of the patient sera. A-ANCA predominated, although not significantly (p > 0.05), in the reactional groups 37.9% vs 20.0%), and in those at the lepromatous pole (41.6% vs 20.0%). There was no correlation between ANCA positivity and either disease duration, disease activity, or therapeutic regimen (p > 0.05). An interesting finding was the correlation between ANCA and gender: 94.1% of ANCA-positive patients were males (p < 0.01), a feature that so far has not been reported in ANCA-related diseases and for which there is no explanation at the moment. By ELISA, the sera of the lepromatous leprosy patients did not show activity against either PR3, MPO, HLE, the most common ANCA antigens. Because A-ANCA are nonspecific, this finding requires further investigation for the determination of the responsible antigen(s). In conclusion, A-ANCA are present in 28.8% of leprosy patients but are not related to vasculitis in the erythema nodosum leprosum reaction and are not a marker of a specific clinical form. (AU)^ien.

Abstract:Estudam caso de hansen¡ase tuberculóide da variedade nodular da infância (Souza Campos), observada em criança de 2 anos e 7 meses, do sexo feminino, branca e que desde o nascimento mantinha contato com o pai, portador da moléstia do tipo V, com baciloscopia fortemente positiva (+++) em L.C. e M.N. Apresentava 4 nódulos localizados no membro superior esquerdo, sendo o maior de 11 X 10 mm e os demais de 4 X 5 mm. Eram duros, indolores, aderentes aos planos adjacentes, de superf¡cie lisa, de tonalidade eritematosa e, quando distendidos, tornavam-se translúcidos e de cor branco-amarelada. O exame histopatológico revelou granuloma tuberculóide (tipo sarcóide) com ausência de b.a.a.r., os quais também foram negativos em baciloscopia de L.C. e M.N.. A reação de Mitsudina foi fortemente positiva (nódulo ulcerado de 8 X 11 mm). Na evolução, as lesões mantiveram-se estacionadas nos primeiros 17 dias; do 35º ao 70º dia houve aumento de tamanho e a partir do 87º dia iniciou-se a regressão. No 125º dia de observação a redução era de 50%. Discutem a influência do fator idade na incidência da moléstia e apresentam os argumentos que justificam a individualização dessa variedade de hanseníase. (AU)^ipt.

Abstract:A brief review of the contradictory opinions of different authors as to the nature and the place of tuberculoid cases of leprosy in the evolution and pathogenesis of the disease is presented. Mention is also made of their failure to agree on what could be generally considered as the esential histological features of this condition. A case of tuberculoid leprosy which has undergone transformation into a lepromatous-type case within six years of observation is presented. This case report together with the clinical, bacteriological and histological studies of other authors seem to agree with the opinion of Manalang that tuberculoid leprosy is a stage in the evolution of leprosy, not immutable as assert by Schujman. That such transformation among adult tuberculoid cases is rare because they represent the filtered resistant ones or benign infections, is indicated. The need for continued follow-up and life-long study of contact children born of leper parents in institutions is indispensable if we are to know the evolution of leprosy. (AU).

Abstract:Numerous clinical and serological similarities exist between lepromatous and dimorphous leprosy and diseases od collagen, particularly lupus erythematosus and rheumatoid arthritis. The clinical manifestations include spontaneous skin-ulcerations, ischaemic necrosis, petechial and purpuric eruptions, vesicle and bulla formation, subcutaneous nodules, hepatosplenomegaly, migratory arthralgia and polyarthritis, bizarre skin lesions including butterfly facial rashes, and a general tendency toward exacerbations and remissions. Haematological findings include anaemia and the ocasional appearance of lupus cells.Positive serological findings include rheumatoid factors, circulating thyroglobulin antibodies, false positive serological tests for syphilis, and cold precipitable proteins (a consistent finding). Because of these similarities, the authors believe that leprosy is a disease which should be considered as a model for the study of hyperglobulinaemic states, especially the diseases of collagen. An important consideration is that, unlike these other diseases, the agent causing leprosy is known.

Abstract:A hanseníase é uma doença infecciosa crônica causada pelo Mycobacterium leprae e que ainda apresenta altas taxas de prevalência nas regiões norte, nordeste e centro-oeste do Brasil. Atualmente, o esquema poliquimioterápico (PQT) preconizado para os pacientes multibacilares tem duração de 12 meses; após o término da última dose esses pacientes recebem alta, sem a necessidade de qualquer avaliação bacteriológica. No presente estudo avaliou-se os índices baciloscópico (IB) e morfológico (IM) de 73 pacientes multibacilares, no momento do diagnóstico e ao final de 12 doses da PQT. Após o término das 12 doses, o IM foi positivo em 20,54% deles, sendo todos virchovianos; esses pacientes continuaram a receber a medicação até 24 meses, persistindo a positividade em 4,11% deles; estes continuaram com a medicação até a negativação do IM. Neste sentido, os autores sugerem que os IB e IM deveriam ser realizados ao final do tratamento e, nos casos de positividade do IM, os pacientes poderiam continuar recebendo a medicação por 24 meses ou mais, evitando, assim, possíveis recidivas e transmissão. A mesma recomendação é feita para pacientes portadores de alguma imunodeficiência. (AU).

Abstract:Leprosy can occur at all ages, in both sexes and in every race. It can be found in any geographic area and can simulate many diseases, especially those affecting the skin. The lepromatous type indicates less host resistance than the tuberculoid type. Histopathologic examination of a properly stained specimen obtained by skin biopsy is the most important diagnostic procedure. Treatment consist of long-term chemotherapy with sulfone drugs.

Tipo Indiferenciado: 1) manchas acromicas anestesicas localizadas por orden de frecuencia en las regiones gluteas, lumbar, muslos y espalda. (AU).

Abstract:Toll-like receptor 2 (TLR2) is a key mediator of the immune response to mycobacterial infections, and mutations in TLR2 have been shown to confer susceptibility to infections with mycobacteria. This study investigated the profiles of cytokines, such as interferon (IFN)-gamma, interleukin (IL)-10, IL-12 and tumour necrosis factor (TNF)-alpha in response to Mycobacterium leprae in peripheral blood mononuclear cells (PBMC) with the TLR2 mutation Argo677Trp, a recently reported polymorphism that is associated with lepromatous leprosy. In leprosy patients with the TLR2 mutation, production of IL-2, IL-12, IFN-gamma, and TNF-alpha by M. leprae-stimulated PBMC were significantly decreased compared with that in groups with wild-type TLR2. However, the cells from patients with the TLR2 mutation showed significantly increased production of IL-10. There was no significant difference in Il-4 production between the mutant and wild-type during stimulation. Yhus, these results suggest that the TLR2 signal pathway plays a critical role in the alteration of cytokine profiles in PBMC from leprosy pateints and the TLR2 mutation Arg677Trp provides a mechanism for the poor cellular immune response associated with lepromatous leprosy (AU).
